Copies of the physical, medical history, and emergency info card can be dropped off at the Anderson High School Main Office to be given to our Athletic Trainers, Coach Busby, and Coach Ngo.

Complete the UIL Forms - the forms will have many links leading to documents for you to review then close, and then a checkmark to acknowledge you understand its terms. At the end of the main form, the athlete and one of the parents/guardians will sign to acknowledge them all. (No need to print any of them to turn in).
